Ushuaia in the Isla Grande de Tierra del Fuego   The largest island in South America, Isla Grande de Tierra del Fuego, is only in 29th place in the world ranking, but there are on the other hand the two largest river islands on the planet.

South America is generally poor in islands, with the exception of the southern part of the continent, in correspondence with Argentina, Falkland, South Georgia and especially Chile, which in fact has almost half of the biggest islands of this continent.

The only other good-sized islands are the Galapagos in the Pacific Ocean and Trinidad in the Atlantic Ocean, which is the only one in the ranking to host over 1 million inhabitants.
