The Sicilian coast near San Vito Lo Capo   The two largest and most populated islands of the Mediterranean Sea, Sicily and Sardinia, which have a fairly close surface area, belong to Italy.

The two southernmost Italian islands, Lampedusa and the smaller Lampione, are part of the African continent; in this ranking there are no islands neither of the Ionian Sea nor of the Adriatic, the largest ones are in fact respectively in Greece and Croatia.
